Fact Information Sheet in Florida: their assets, in their words

General information, not legal advice. Ask a lawyer before you act on it.

A Fact Information Sheet is a form the other side fills in about their assets after you win. In Florida you ask the court to enter an order directing them to complete it; the clerk lists it first among the ways to collect.

You ask
The court, for an order
They fill in
Their assets
Then
A writ or a recorded lien

The steps

  1. Ask for the order.

Request that the court enter an order directing the judgment debtor to complete a Fact Information Sheet, which may help identify assets. Source

  2. Read it for what a writ can reach.

A Florida writ of execution works on property you can locate and describe: a vehicle, an account, equipment. Source

  3. Record where they own real estate.

A certified copy of the judgment recorded in that county's Official Records can make a lien on the property. Source

  4. Register, then go to the sheriff.

With the Judgment Lien Certificate, the writ and Instructions for Levy. Source

A worked example

If the sheet lists a 2019 pickup registered in their name and a home in Seminole County, that is two moves: record the judgment in Seminole County's Official Records for the house, and write Instructions for Levy naming the pickup and where it is kept for the writ.

Fees and forms change, and your court's clerk has the current ones. General information, not legal advice. Ask a lawyer before you act on it.

Ask once, in writing

Before the court steps, most winners send a letter asking for payment by a date. Fill one in and print it; nothing you type leaves this page.

Does the clerk send it for me?
